Shaak Ti may hold the strangest death record in Star Wars. She was killed by General Grievous, stabbed by Anakin inside the Jedi Temple, and later faced another death in Legends. Each version tried to give her a final ending, but another story always replaced it.

Shaak Ti Was Originally Killed By General Grievous
Shaak Ti’s first filmed death was meant to happen aboard the Invisible Hand during the opening of Revenge of the Sith.
While Anakin and Obi-Wan were searching for Chancellor Palpatine, they found Shaak Ti being held captive by General Grievous. She had already been beaten and restrained, and Grievous used her to taunt the two Jedi before suddenly driving a lightsaber through her back.
Anakin and Obi-Wan were forced to watch her die before continuing their pursuit of Grievous and Palpatine. The scene would have made Shaak Ti one of Grievous’s most important Jedi kills and shown how dangerous he was before their later duel aboard the ship.
But the entire sequence was removed when the opening rescue mission was reworked. Shaak Ti’s death by Grievous never appeared in the finished movie, and another death scene was created for her later in the story.
Anakin Was Given Her Second Death Scene
After the Grievous version was removed, Revenge of the Sith gave Shaak Ti another death during the attack on the Jedi Temple.
In the deleted scene, Shaak Ti is meditating inside the Temple while Anakin approaches from behind. By that point, he has already turned to the dark side and entered the Temple with the 501st Legion. Without warning her, Anakin ignites his blue lightsaber and drives it through her back.
This version placed her death directly inside Order 66 instead of aboard the Invisible Hand. Shaak Ti would have survived the Battle of Coruscant only to be killed by another Jedi inside the place she was meant to protect.
That scene was also removed from the finished movie. Shaak Ti had now been filmed dying twice in the same production, but neither death appeared in the theatrical cut.
Legends Killed Her Again Before Canon Chose Vader
In Legends, Shaak Ti survived Order 66 and escaped to Felucia, where she protected the planet’s Force-sensitive inhabitants and trained them to resist the Empire.
Years later, Darth Vader sent his secret apprentice, Starkiller, to eliminate her. Shaak Ti fought him beside the Great Sarlacc and nearly defeated him, but after losing the duel, she fell into the creature’s mouth and died. This became her official Legends fate in The Force Unleashed.
Canon later returned to the Jedi Temple version. During Yoda’s vision in The Clone Wars, Shaak Ti is shown meditating as a blue lightsaber pierces her from behind. Her recorded message in Rebels also identifies Anakin Skywalker as the attacker during the Temple massacre.
